Project Information:

The Chronic Care Technology Implementation Project (CCTIP), was originally funded by Agency for Healthcare Research and Quality (AHRQ) through a grant awarded to The Aroostook Medical Center (TAMC).

The project's mission is to improve healthcare management in northern, eastern, and central Maine by planning for a standard exchange and integration of clinical information among healthcare organizations through the use of technology by:

    • Creating a plan for rural Maine for the sustainable use of health information technology that produces safer, higher quality, effective and efficient delivery of health care for patients with chronic disease.
    • Establishing the necessary infrastructure to a comprehensive, integrated information technology network for communication in support of patients with chronic disease.
    • Adopting and improving processes to improve patient care coordination and to facilitate availability of clinical patient data among healthcare organizations that is consistent with federal, state, and local confidentiality regulations.

This project was supported by grant #UC1HS016154 from the Agency for Healthcare Research and Quality. The content is solely the responsibility of the authors and does not necessarily represent the official views of the AHRQ.
